In the context of the social prescribing pathway, an individual referred to a social prescribing practitioner<\/a> co-produces<\/a> an action plan<\/a> following a what matters conversation<\/a>. Within this context, the term co-design<\/strong> is occasionally used interchangeably with the term co-production<\/strong>. However, co-design<\/strong> specifically refers to a mutual relationship between service providers, service users and their families and communities in the context of the design of services.<\/p>\n
