Mae presgripsiynu cymdeithasol yn fath o gymorth sy'n helpu i gysylltu pobl \u00e2 chefnogaeth gymunedol i reoli eu hiechyd a'u llesiant yn well.<\/strong> Gall helpu pobl mewn sawl ffordd a rhoi mwy o reolaeth iddynt dros eu hamgylchiadau.<\/p>\n\n\n\n

Gall bresgripsiynu cymdeithasol helpu gyda llawer o bethau, fel unigrwydd, gweithgarwch corfforol, problemau cyfreithiol a chyllid. Efallai y bydd person yn cael ei atgyfeirio at ymarferydd presgripsiynu cymdeithasol gan rywun fel gweithiwr gofal iechyd proffesiynol (fel meddyg teulu) neu weithiwr cymunedol, neu efallai y byddant yn penderfynu ar eu pen eu hunain i gysylltu \u00e2'r ymarferydd presgripsiynu cymdeithasol.<\/p>\n\n\n\n

Gall yr ymarferydd presgripsiynu cymdeithasol sgwrsio \u00e2 nhw i'w helpu i adnabod eu hanghenion anfeddygol a chael mynediad at gymorth lleol. Efallai y byddant hefyd yn eu cefnogi drwy fynychu'r gweithgarwch cymdeithasol wedi\u2019i atgyfeirio. Er gwaethaf defnyddio'r term 'presgripsiynu', nid yw presgripsiynu cymdeithasol yng Nghymru yn golygu bod rhywun yn dweud wrth rywun beth ddylent ei wneud. Mae'n ymwneud yn fwy \u00e2 chefnogi rhywun i adnabod meysydd o'u bywyd a'u llesiant y maent am eu gwella.<\/p>\n\n\n\n

One problem with social prescribing is that it has a lot of confusing language. To make it even more confusing, some of the aspects of social prescribing use different terms to describe the same thing! \u200b<\/p>\n\n\n\n

This can make it hard for the people who work in social prescribing to communicate with each other. It also makes it hard for them to communicate with members of the public. This can prevent people from using social prescribing. After all, if you can\u2019t understand what something is, you might be less likely to give it a go. <\/p>\n\n\n\n

\u200bFor this reason, we\u2019ve made a glossary of social prescribing terms to make it easier to understand.\u200b\u200b<\/p>\n\n\n\n

Beth yw rhestr termau?<\/strong><\/h3>\n\n\n\n

Gallwch feddwl amdano fel geiriadur ar gyfer presgripsiynu cymdeithasol. Mae'r rhestr termau yn deillio o lawer o ymchwil. Yn yr ymchwil hon, gwnaethom adnabod cannoedd o dermau a ddefnyddir o fewn presgripsiynu cymdeithasol. I gael gwybod mwy, ewch i\u2019r dudalen \u2018Creu'r Rhestr Termau\u2019<\/a>. O'r termau hyn i gyd rydym wedi llunio rhestr o 36 term sy'n disgrifio'r agweddau pwysicaf ar bresgripsiynu cymdeithasol.<\/p>\n\n\n\n

Efallai eich bod eisoes wedi clywed am rai o'r termau yn y rhestr hon ac wedi meddwl \u2018tybed beth maen nhw'n ei olygu\u2019. Mae termau eraill yn fwy tebygol o gael eu defnyddio gan y rhai sy'n gweithio ym maes presgripsiynu cymdeithasol yn unig. Cynhyrchodd WSSPR<\/a> ac Iechyd Cyhoeddus Cymru<\/a> y rhestr termau a anelir at y rhai sy'n gweithio mewn, neu gyda phresgripsiynu cymdeithasol. Rydym wedi cynhyrchu gwahanol fersiynau o'r rhestr termau i sicrhau bod hynny'n hygyrch i weithwyr proffesiynol a'r cyhoedd.
